Long-term care facilities (LTCF) staff and residents
Available around January 11, 2021, to:
Adults 65 and older
First Responders (fire, police, sheriff, 9-1-1 personnel, etc.)
We will begin taking contact information for Adults age 65 and over and their caregivers and First Responders who are interested in receiving a vaccination around January 11, 2021.
Can I get a vaccination?
At this time, we are cleared by the Georgia Department of Public Health to provide vaccines within the Phase 1A group only. Practitioners and staff of large medical groups or hospitals who will receive their own supply of vaccines should get their vaccine through their employer.
If you are an individual healthcare provider or long-term care facility provider, resident or worker, or you need to have your entire practice receive the vaccine through our agency, please complete the information in the form below.
